Don't send address by default

Once an address is set in the user's profile, new requests will attach it by default. However, (from my experience) most public bodies accept FOI requests without a postal address, now.

I'd like to suggest adding a new checkbox along the address field to selectively enable sending the address with new requests, like it's already available in the form to send an answer. This checkbox would be unchecked by default, but users could select a personal default in the profile's preference pane.

If this feature-request is accepted, I could contribute the necessary changes as PR.
