Adds source_url; adjusts kibana

This PR has several new features:

  • adds source_url to feature_association
    • populated for [civic, jax, cgi]
  • kibana heatmap names adjusted
  • associations grid shows source_url

CGI + MolecularMatch should have a static URL here instead of nothing. CIViC should use shortlinks, e.g. https://civicdb.org/links/evidence/1045
