Add SOP docs for all uPheno social workflows

I created a skeleton for this, if you could flesh this out with as much details as possible.

A Phenotype Ontology Editors' Workflow document has been created that I think covers the "Patternisation" section of reconciliation.md. I will look into how to better integrate these documents.
