Requested custom icon is returning randomly in production site

This bug is so random and it's only happen on our production site. Sometimes it happen, sometimes not. I tried create a reproduction but it's only happen on our production site. But you can see from the screenshot below that the request to times icon is returning 2 other icons. It return the error message: [Icon] failed to load icon m-icon:times
