Email users when a comment is added to their content

When a comment or feedback stamp is added to a question or exam, anyone with edit access should be emailed.

There should be an option to unsubscribe from emails for a particular item or a whole project.

The big risk here is that people will use this for spam. That's what's putting me off implementing any kind of email notification.
