Combined the Manual into 1 file, added html and pdf versions

It was hard to go through different manual files while I was trying to learn this plugin. So, I added the content from all the md files to Manual.md and made sure that the "Table of Contents" links take you to the correct section. For ease, I also added the HTML and PDF versions of the combined Manual. Hope this helps people.
