support doctest file fixtures, try to use the same format as sphinx

Similar to the feature in nose or in sphinx's doctest support -- actually now that I think of it we should try to be fully compatible with sphinx so that nose2 can run sphinx-style doctests. Maybe sphinx's doctest extension can actually be reused?
