Fix cui_to_entity indexing bug

Fixes the first expected behavior mentioned in #20.

I'm not sure that this fix is universal; I'm confused about how the entity linker works when embedded in the full pipeline using ncbi_taxonomy, but then fails here when used alone. Would love your feedback on whether this is actually an expected behavior, and how to deal with it if it is.
