Enhancement - Tidepool uploader should include notes

In AAPS version 3.2 Tidepool uploader doesn't include notes written in AAPS.

I think that notes should be reported to Tidepool in all cases where it is possible to write notes in AAPS:

  1. Bolus Wizard screen
  2. Carbs screen
  3. Insulin screen
