camera opens and shows black screen when use of camera "not allowed"

First, thank you very much for the framework.

I have encountered a small issue regarding the use of the camera when changing an image in your FXForm. I am using an iPhone 5 and iOS8.3.

To reproduce, do the following:

1. Select an FXFormFieldTypeImage and press on it.
2. Disallow the usage of the camera when prompted (or go to settings and disable there).

This results in the camera being opened, but with a black screen.

The good thing is, that the app doesn't crash. The bad thing however, is that the user of the app has to realize for himself, that his permissions for the camera usage are wrongly set.

A fix for this would be very appreciated. Thank you in advance.
