Are you referring to creating a .nomedia on your device's filesystem or in your Nextcloud Server hosted folder structure?
On nextcloud. The folders are not in Android.
I have the same problem. NC App ignores the servers nomedia file, when going to the Media section.
Tested this on a Pixel 4A, Android 13, Nextcloud Android App Version 3.27.0 In "Media" pictures are shown even if their folder itself contains a ".nomedia" File. Additionally, when swiping through the pictures after clicking one, pictures come up that aren't shown in the overview but are located outside the media folder (selected in the triple-point menu).
This is a major issue that I'd like to see addressed. Similar problems with Nextcloud on iOS: nextcloud/ios/issues/1474
