Update documentation for using Azure ARM templates

At the very least, this story covers adding/updating our docs for installing the agent using an Azure ARM template with the site extension.

If the spike #685 shows that we do in fact work with PackageReference nuget, then this story will include expanding the docs to cover the install process for that as well.
