No fault icon is being displayed

When label for temperature is being set, then no fault icon is being displayed:

NOT OK: Label temperature set and no fault icon is displayed: image

OK: Label temparature not set and fault icon is displayed: image

I was expecting that specifying the entity only would use its default icon. However, I find that the icon is only shown if it is specified in the card's config.
