Compability with wagtail 2.12

I updated the code according to

• https://docs.wagtail.io/en/stable/releases/2.2.html#javascript-templates-in-modal-workflows-are-deprecated
• and removed the per_page parameter which seems to be supported no more

this should resolve #29
