Add a 3D model

Although it seems a bit late to use this model now, I still believe it could be helpful to someone.

This printable model is designed for MCE Adapter v0.7.1.

There are two important points to note:

Use a 0.1" pitch slide switch for J4. Use a 0.1" pitch 6-pin DIP angled switch for J1-3.
