Incorrect mapping usually means the controller is being detected as something else. I will need to see the vendor/product id to see how the controller identifies itself.

I assume this means you tried to run btdb to get the vendor/product id? This happens on firmware 17.0.0+ because there are no longer enough free handles to the btm service the application uses to query the pairind database. You need to temporarily disable mission control in order to run the app.
