`Reader#each_entry` includes "pax_global_header"

I don't think we want to have `pax_global_header` considered as a normal file? I propose to ignore it.

See for example: https://naqvis.github.io/crystar/Crystar/Header.html  A lot of these methods have the wrong documentation because the comment is at the end of the line instead of the previous one.
