Remove dependency on `error-chain`

Rewrite the errors to not use the deprecated and defunct error-chain library.

This issue will be celebrating a 5 year anniversary this September. I think we should order a cake and some champagne. 😆 On a serious note, is there any plan to get rid of error_chain?
