What combination of pidgin notification plugins does this work with?

Should I be using the Pidgin Libnotify or message notification popups in combination with this? I can't seem to get it to kick in...

No special plugins is needed. The extension is using D-Bus.

Hm. Is it known not to work for group chats?

There are option "Track only highlighted messages for chats" in the extension preferences. With this option enabled (by default) notifications for group chats will appear only for highlighted messages.

Thanks, I've got it working now! Is there a known working way of also suppressing the pidgin application windows from popping up when notifications are sent to gnome (i.e., the stock empathy behaviour), so that pidgin application windows are only summoned by clicking through on the notification?

Try to set the "Hide new IM conversations" option in the pidgin preferences to "Always".
