When uploading all I get is the title and the black screen

I love this form of uploading and I'm so close to having it setup, it works until you need to actually upload something. All I get is the title and the black background, any fixes?

I just fixed by adding the tag "< ul id="simpleupload-ul">" (remove the quote marks) under the simpleupload-form ;)
