Prevent rezzing ice (with the rez ice button) except when it's relevant

I think our engine has come far enough that we can restrict rezzing to:

• when the ice is being approached
• abilities
• whatever rime does
• commands

Having said that, this might also be better off waiting until we implement priority windows (on my radar for the next year).
