24 Ch mixer - full negative voltage sometimes output on random channel

I was working on a track last night and when I connected a snare drum to Channel 5, that channel immediately started outputting constant full negative voltage. Anything I try to put into that channel now results in the same negative voltage - no way to stop it other than not having anything plugged in to it.

I asked on the FB group and another user has the same issue - says it can randomly happen on any channel - even on the main out sometimes which means you can completely lose a mix as there seems to be no fix other than deleting the mixer from the session.
