Can we add build instructions to the README? I'm not sure what's the recommended/canonical way to produce the book. I have some ideas how I can though with pandoc.

chapters 6 & 7 don't match the updated site

According to the site, chapter 6 is Debugging and chapter 7 is revisions. It seems there was an error with them not showing up which has since been fixed.

Suspected typo in line 25 "grep/ack" where "grep/awk" makes more sense.
