File upload on SharePoint 2019 modern site

I have deployed the package on SharePoint 20919 modern site, however file upload is failing. Any guidance will be really appreciated. Below are the specific configuration for file upload:

On the document library Create Column Name : orderlistlook Get Information from : Orderlist (This is the Stratus Form list name) In this column (ID)

How did you deploy to Modern? Are you using the SPFx web part I created or did your roll your own?

@mrackley I am using the SPx web part you created from this link http://www.stratusforms.com/spfx/1.55/stratus-forms.sppkg

When I click the submit button, the page content is submitted to the List but the attachment is not sent to the Document Library settings_Sharepoint

@mrackley did I configure it correctly or possible workaround for the File upload?
