Choose a canned response from a list when commenting

When adding a canned response comment (https://github.com/mozilla/addons-code-manager/issues/113), the user first needs to select a response from a list.

If the list of canned responses is short enough, this could be a select box. As an enhancement, we could add autocomplete / searching.

One of the choices should be other which will switch to text entry (https://github.com/mozilla/addons-code-manager/issues/112)
