Add metadata to extraction outputs: model name and datetime

Extraction (SPIRES) output should include model name (including source), time, and date. It may be useful to include whether the query was run vs. live API or vs. cached result, too. The general goal here is reproducibility and including sufficient provenance for comparison, particularly between different models and model versions.
