MCIP Draft - MCIP Process

Introduces a new MCIP process based on BIP-0002, and refactors the MCIPs repository to support it.

In accordance with the existing proposal process, this proposal should only be merged if it should be given 'Active' status (i.e. if it should replace the existing proposal process).
