Vault Mount Cleanup

We need to cleanup unused / disabled vault mount points that are deprecated and won't be supported in future versions.

I believe if we go into the vault UI and the mount list, at the bottom of each instance are greyed out mount points. We should confirm that each is not in use and then force revoke all leases and disable the mount point.
