User Port: Make independent of KERNAL user port code

The User Port RS232 driver currently calls into the KERNAL to set up some registers. Some modified versions of the KERNAL, like DolphinDOS and SpeedDOS have the RS232 code removed, so the User Port driver probably doesn't work with them.

Instead, we should do all necessary user port setup inside CCGMS.
