Plugin disappears from menu after I try to add tilemap to layer

After I tried to select a load a tsv file from the web > tilelayer menu option (which didn't work, although that was probably my fault), so I tried to go back and fix it, but the item is now gone from the menu and I can't get it back without uninstalling QGIS completely AND deleting the registry data.

What OS and which version of QGIS are you using?

Windows 7 Pro 64-bit, QGIS 2.8.2-1

same thing just happened to me, Mac OS X 10.10.3, QGIS 2.8.1 -- I tried to add a tile source, and now I can no longer see the plugin even after uninstalling and or reinstalling it.

Haven't you checked Move plugin to Layer menu/toolbar option? If checked, plugin menu item is in Layer - Add Layer sub-menu.
