Annotate Generated Responses With Model Name in Both Chat and Edit

Please annotate the generated responses with the model name in chat and edit screens. This will be beneficial as it'll help users better differentiate model response qualities.

@roblourens, I guess we can close this now since the feature seems to have been implemented in the chat screens.
