Yes it is related to this Sample App's Cosmos DB history feedback feature.

Is your feature request related to a problem? Please describe.

Currently users are not able to explain more detail about their negative feedback. We are having to track user feedback separately. Having the ability to keep all the feedback in in one database would be beneficial for all.

Describe the solution you'd like Adding a text box to the "Submit Feedback" box to allow users to input additional information or links to correct documentation. Something like this example: Current Screen:

Is this feature specific to your use case or your organization, or would it apply broadly across other uses of this app? I believe everyone who uses this app would benefit from this feature.

Describe alternatives you've considered There are no alternative solutions that make more sense than allowing input directly from the feedback window.
