Add AppInsights doesn't use Azure credentials to let me pick an app Insights resource

Describe the bug

Add AppInsights just brings up the form, it doesn't have a resource picker

To Reproduce

1. Add AppInsights

Expected behavior

To be able to enumerate and pick the app insights resource using my credentials

1. to select from a alphabetically sorted list of tenant names and have that selection be sticky
2. to select from a alphabetically sorted list of subscription names and have that selection be sticky
3. to select from a alphabetically sorted list of resource group names and have that selection be sticky
4. to select from a alphabetically sorted list of cosmosDB storage accounts
