New readme for composer form dialog (composer-form-dialog.md) and updates to generated dialog readme (readme.md). Written by Melanie Wang.

Hi @tracyboehrer, this PR is quite old, and the form dialogs feature is documented in Composer: https://github.com/microsoft/BotFramework-Composer/blob/main/docs/preview%20features/form-dialogs.md Additionally, there's no composer sample for this feature. I think we can close this PR.
