Migrate to UMD and extract CSS to avoid unnecessary conflicts with SSR

Changes:

  • Migrate from commonjs build to UMD
  • Extract CSS to its separate file (because styles injection caused issues with SSR, especially Nuxt 2)
  • Update docs to reflect changes
