How to click the middle (scroll wheel) button?

I see an interface for scrolling the scroll wheel, but there does not appear to be one for click the button on the scroll wheel. There are references to a MiddleButton in the code, but as far as I can tell, this does not appear to be currently supported. Can someone please confirm or correct me? If it's not possible with InputSimulator, is SendInput or mouse_event recommended?
