It should be a manually triggered workflow

It could also be triggered once a year and it could open a PR. The PR can be manually reviewed and if necessary, changes could be made.

Everything is possible. When it will be triggered is a matter of 2-3 lines updation. Manual is one of the option it should have. There can be a cron job as well. That is both can exist simultaneously.
