Better support for addresses/referrals indexes

Problem

Right now we have address and referrals indexes that are required for block validation and number of rpc methods.

We need to make them optional and update validation code to use minimal amount of data that is flushed to disk when all indexes are turned off.

Also need to take #222 into account to get back periodic flush
