Tutorial and Documentation Clarification

In the last medspacy release, we improved our documentation by adding docstrings and type hinting, however documentation is not 100% complete and does not represent all behavior that medspacy is capable of. Need to start longer process of clarification and adding more detail.

#166 - clarify matching rules and features added by medspacy vs limitations of spacy patterns #159 - spacy span group visualization #190 - clarify functionality relating to creating/modifying rules specifically without using rule objects
