Allow skipping content-type header if already set

Slightly related to https://github.com/mdirolf/nginx-gridfs/issues/4, when using X-Accel-Redirect, could we have some option that allows us to use the original Content-Type header that we already set, and ignore the one in GridFS?

this makes sense, but i don't think i'll have the time to get to it soon. if you feel like having a go at it i'd be happy to merge!
