Notifications of PV creation and destruction by PVA Server

This feature request spins off a discussion at the EPICS Codeathon today at Diamond.

I would like to implement a Python version of RecCaster to feed data to ChannelFinder about PVs created via PVA Server. It would be convenient to be able to subscribe to notifications about the creation and destruction of PVs so that the RecSync data can be kept fresh.
