Minor tweaks to README.md

Removed duplicates and added install instructions so others can just copy and paste to install. I also like code blocks and I think it makes the repository look cleaner.

[Shodan.io](https://addons.mozilla.org/en-US/firefox/addon/shodan_io/) can't work properly right now, so I changed it to [Shodan](https://addons.mozilla.org/en-US/firefox/addon/shodan-addon/) which is a official Shodan add-on. You can see the details on [PaulSec/Shodan-Firefox-Addon issue#13](https://github.com/PaulSec/Shodan-Firefox-Addon/issues/13)
