Something like this (or better):

a "Bridge Service" is responsible for a subset of bridged rooms (plumbed/portal) to handle both Telegram and Matrix traffic

for clarity: the puppet and bridge groups in the diagram should be independently scaled, to allow for more rooms (bridge services) or more puppets (logged in people) to be handled. A similar third layer could be added for syncing Matrix users, although for my particular use case that doesn't matter as much.
