Update syntax for using contact tokens to pre-populate forms
I believe after testing that the "&" should actually be a "?". There is a lot of chatter on the web about why this format does not work. People are finding ways around it like using the page url in place of the short page code. But if we correct the manual,(assuming I am right) we can save a lot of people the problems I have had all day.
@interactivewebs thanks for making this PR! Would you possibly be able to add some short instructions on how to test if this is indeed correct?

To be honest I am not a coder. I messed around and simply found that when testing following the instructions in the manual, that the result was a big fat 0. So I looked for chatter on the net about this. One suggestion had a different format that replaced the & with the ?. So I tested and found that the result was a perfect test.
I would be quite sure that someone with coding skills could take a look the project and in about 5 minutes confirm programatically what I am saying here.
