Remove the lookup_hash field from local_threepid_associations

https://github.com/matrix-org/sydent/pull/184 added a lookup_hash field to local_threepid_associations but this isn't actually needed.

Remove it in a new migration step and make sure 3PIDs are just hashed upon local and global replication.
