slug doesn't update with title change before publish

if you start a post with one title, save (or create via the wee input on admin page), then edit it and change the title the slug doesn't change.

it should change when you change the title and it is either still a draft, or being published during the same save

if (posted_at) 
   don't change slug 
   UNLESS someone has manually changed it
