I can only otherwise suggest doing a search in your Windows Registry with "Extreme Injector" (without quotes) and deleting any offending entries. There may be some stuff that gets added...
Have you tried the checkbox that says “Use alternate tile map server”
Do you have access to a different network just to rule out any block on your IP address? Maybe if you have mobile data you can hotpot your 4G/5G connection...
What OS are you on? If you are on Windows, can you check what the black console window says?
