EHR. Create Demo Visits throws "Error creating appointments"

Tested on: EHR Staging 1.6.3

Steps To Reproduce:

  1. Enter valid phone
  2. Click on "Create Demo visits". ER: 5 In-person and 5 Telemed appointments are created. AR: Appointments are not created. "Error creating appointments".
