Support Tool annotations

As outlined in the spec https://modelcontextprotocol.io/docs/concepts/tools#tool-definition-structure

They have been addressed in https://github.com/mark3labs/mcp-go/pull/158 but I can't see them in the response of the MCP client. There seems to be an issue.
