[feature]: Customizable Intake Forms

Summary
Can we get a customizable intake form, perhaps similar to the custom Work Item Types, where a form can have a custom title, description section and list of custom input fields with titles and response types (maybe free text with specified height and perhaps images, date, email address, files, numbers or booleans), and with "required?" and "active?" flags for each?
Why should this be worked on?
Currently the standard intake form says: "Create a work item; Let the team know what you would like them to work on.", with text boxes for "Name", "Email", "What's this work item about", and "Describe what should happen".
Someone who understands the process intimately (likely on the small team of project/product/process managers using plane) can fit their request to this if it doesn't fit the problem that they intake form is used for. However, someone who does not have the context of the management of the project/product/process will not have a good appreciation for the information required to create a good ticket, so the tickets may not be informative.
The form should allow managers to provide more guidance and context to users filing tickets so that they can create good tickets.
Will this have the ability to select which work item type is requested? Could this just be a 1:1 of a custom work item?
Hi @the-sean-c thanks for raising this request. A customizable intake form is already on our roadmap and will be rolling out in one of our future releases.
Thanks @pushya22 ! I've put in a couple of feature requests, and I hope you understand that I mean it as a compliment: I'm taking the time to do it because I think plane is frickin' awesome! I've got my whole team on it!

Hi @leedsjb - this looks cool, but it's not quite what I'm looking for. I want to use forms for bug reporting, so I'm looking for customization of the fixed text and fields on the forms. For example, the subtitle right now is "Let the team know what you would like them to work on." and the placeholder text in what's this work item about? is "Approve laptop purchase", which I have no control over.
I want to be able to control all the fields and text on the form, so that someone who is not on my team knows what the form is for and what is supposed to be put in it.
